passion.PRIMARY PURPOSEResponsible for learning special skills and
competencies in a specific area of pharmacy practice while
participating in a twelve-month program of structured post-graduate
education and service experiences in order to gain advanced
knowledge of pharmacy practices. MINIMUM SPECIFICATIONSEducation:-
Must have a Doctor of PharmD degree from an accredited school of
Pharmacy.Experience:- Completion of an ASHP-accredited PGY1
residency training programpreferred.
Certification/Registration/Licensure:- Must be a registered
pharmacist in the State of Texas or become registered within the
initial three-month appraisal period.Skills or Special Abilities:-
Must be able to demonstrate knowledge of drug therapy, drug
distribution systems of the pharmacy, and formulary management
skills. - Must be able to demonstrate knowledge of all drugs,
procedures, and equipment used in the operations of the pharmacy.-
Must have good oral and written communication skills.- Must have
good organization skills. - Must be detail oriented. - Must be able
to demonstrate leadership skills. - Must be able to demonstrate
cross-cultural sensitivity. - Must be able to demonstrate knowledge
and skill in meeting age-related and handicapped patient's needs. -
Prefer ability to speak a second language.Responsibilities1.
Attends training and develops competency in a specialized area of
pharmacy practice which includes patient care, drug information and
drug use policy, and practice management to improve knowledge of
pharmacy services and practices.2. Develops professional practice
patterns, habits, and a level of accountability exceeding that
obtained through pharmacy practice residency or equivalent
experience.3. Educates providers (M.D., N.P.), peers, nursing and
medical staff residents, and specialty residents to enhance patient
safety and reduce drug costs through their clinical interventions,
in-services, newsletters and presentations.4. Educates patients to
increase compliance, medication and self-care knowledge and
awareness of role of pharmacists in their care.5. Extends Pharmacy
DepartmentAs resources through staffing to include: a. Evaluates
orders and prescriptions written by medical staff and other
authorized prescribers. Performs prospective comprehensive review
of patientAs prescription orders and profiles at the point of
dispensing in order to identify potential drug related problems. b.
Actively participates in resolving identified potential drug
related and other problems that may improve the outcome of patient
care. c. Responsible for monitoring the compliance of formulary and
restriction programs. Provides relevant feedback and input into
safety and process improvement within all pharmacy areas to improve
medication error prevention. d. Serves as resource person,
answering all questions from physicians, nurses and patients
concerning pharmacy departmental policies and drug therapy to
ensure that all healthcare professionals are informed of the
necessary policies, procedures, and information critical to optimal
patient care.6. Identifies, organizes, implements, and individually
analyzes a major residency project which contributes to the
enhancement of pharmacy services. Attends and presents residency
project at state, local, and national professional meetings to
update skills and interact with other professionals within the
practice of pharmacy.7. Attends Pharmacy and Therapeutic Committee
meetings to aid with formulary projects (i.e. monographs) and give
recommendations for formulary enhancements.8. Expands new or
existing services to enhance capacity and reach of clinical
pharmacy services.9. Assumes responsibility for own professional
growth and development by attending seminars and workshops, reading
professional journals, maintaining professional affiliations, and
maintaining current registration as a pharmacist in the State of
Texas in order to improve professional knowledge and maintain
certification required by law.10. Maintains knowledge of applicable
rules, regulations, policies, laws and guidelines that impact the
Pharmacy area. Develops effective internal controls that promote
adherence to applicable state/federal laws and the program
requirements of accreditation agencies and federal, state, and
private health plans. Seeks advice and guidance as necessary to
ensure proper understanding.Parkland Health and Hospital System
